Taking baseline eye examination before starting plaquenil

In Sjögren’s, Plaquenil is used to treat many symptoms of Sjögren’s including fatigue, joint symptoms of arthritis and arthralgias (joint pain), dry mouth and dry eyes. Similar to its use in systemic lupus erythematosus, many clinicians feel that it is useful in reducing general Sjögren’s “disease activity.” One of the reasons that physicians feel comfortable in prescribing Plaquenil is its low risk to benefit ratio.

Plaquenil/Hydroxychloroquine - which eye tests do you need and when. One of us posted detailed info about her experience & the different types of eye test anyone on plaquenil etc would probably be best to have, and the frequency. Baseline eye screen and plaquenil; Friends.hydroxychloroquine, what do you think? I was told from the start by my rheumatologist I would have to have eye exams including visual field exams every six months due to the plaquenil therapy. My first two exams did include these tests. But my next six month appointment is coming up, and they told me I didn't need the visual field test this time so the appointment would be shorter. An eye exam is recommended at the start of treatment to detect any existing vision problems; follow-up tests are needed to catch retinal damage in the earliest stages, before symptoms appear. If found later, damage to the eye may be irreversible, even if the medication is stopped.

As with any medication, allergic reactions including skin rashes and non-allergic reactions can occur. This means that the side effects of Plaquenil are mild and infrequent compared with its potential benefits.

Baseline Examination All patients should have a baseline ophthalmologic examination within the first year of initiating HCQ or CQ therapy with thorough risk factor assessment and fundus examination of the macula to rule out underlying disease. Baseline visual field and SD OCT are recommended if abnormalities are present e.g. macular lesions, glaucoma, etc that could affect screening tests. All individuals starting Plaquenil treatment should have a baseline ophthalmologic examination within the first year. If the initial ophthalmologic examination is normal and the person is considered low risk, re-testing need not occur for five years. It is recommended that high-risk patients have annual eye examinations. The changes are most often permanent, but in some cases, vision has improved. Most rheumatologists recommend patients undergo a baseline eye examination prior to starting Plaquenil and be re-examined in the future depending on their risk for developing the condition. Risk factors include advanced age and having pre-existing retinal disease.
